Hauora Hub & Mobile Nursing
A place to connect whaanau to the right services and nurses who visit whaanau at home for ongoing care.
Hauora Hub
The Hauora Hub is the place where whaanau start when they need help from Whare Tiaki Hauora. It helps make things easier by bringing services together, so whaanau don’t have to tell their story over and over again.
When a whaanau contacts us or is referred, the Hub team listens first. We ask a few simple questions to understand what is going on. We then help connect the whaanau to the right team or teams who can support them. This might include health care, mental health support, housing help, nursing, addiction support, or social support.
The team that works with you takes time to get to know you and your whaanau. Together, you make a simple wellbeing plan based on what you need and what matters to you. We also look for other support that could help — even things you might not know about yet.
The Hauora Hub works closely with our own teams and with other services in the community. This means everyone works together, and whaanau get the right help at the right time. Our nurses, kaiaawhina, social workers, and clinicians talk with each other to make sure care feels joined up and clear.
We created the Hauora Hub because many whaanau told us the system feels confusing and hard to navigate. The Hub helps make the journey simpler, safer, and more supportive.

Mobile Nursing
Our Mobile Nursing & Hauora Hub brings healthcare to whaanau. We come to you, so you don’t always have to go to the clinic or hospital. We help whaanau who have ongoing health needs to stay well and feel supported at home.
Our nurses help with health checks, long-term health problems, medicines, and immunisations. They take time to listen and explain things in an easy way. They also check in between doctor visits to see how things are going.
We work with your GP, hospital teams, and other services so your care feels joined up. We also look at what’s happening at home, like food, warmth, and safety. If you need extra help, our kaiaawhina can connect you to housing support, Whaanau Ora services, Work and Income, and cultural support.
